It's under it.
Only a little of an accident stays on the surface. The rest soaks straight through and sits in the underlay underneath, where you can't see it and a normal clean never reaches it.
Sprays and powders only touch the top. That's why the smell fades for a few days and comes back, and why it's worse in warm or damp weather.
Reach the underlay and the smell goes. Treat the surface and it always returns.

These are harder, or not possible
- Sometimes urine has permanently changed the dye. When that has happened, no cleaning brings the colour back.
- Years of accidents in one spot may need a second visit.
- If it has gone into the floorboards, the carpet may need lifting. Rare, but it happens.

It depends on how many spots there are and how far in it's gone, which is why I look first. The minimum call-out is £120, and you get the full price before I start.
Usually, yes. A dog goes back because it can still smell itself there, long after you've stopped smelling anything. Take that away properly and the habit normally stops with it.
Often, yes. A lot of yellow marks lift out completely and the carpet goes back to its original colour. Sometimes the urine has permanently changed the dye, and when that has happened nothing brings it back. I can usually tell you which one you're dealing with when I look at it.
Almost never. The tool I use pulls the liquid up through the carpet from the underlay below, so it stays where it is. Only in extreme cases would lifting it be worth considering, and I'd talk that through with you first.
